Latar Belakang: Angka obesitas di Aceh meningkat, seiring minimnya literasi gizi dan rendahnya efektivitas pendekatan konseling konvensional. Pendekatan digital dinilai potensial untuk meningkatkan asesmen dan edukasi gizi. Penggunaan aplikasi digital sebagai media konseling gizi diharapkan dapat memberikan dampak positif dalam pencegahan dan pengendalian obesitas.
Tujuan: Penelitian bertujuan untuk mengembangkan model asesmen dan kebutuhan gizi berbasis aplikasi mobile “ObesiCare” untuk pencegahan obesitas melalui konseling gizi dengan pendekatan Transtheoretical Model (TTM) dan Model 5A’s di Provinsi Aceh.
Metode: Penelitian mixed-method (desain research and development dengan quasi-experimental) ini terdiri dari studi eksplorasi, pengembangan aplikasi Obesicare dengan pendekatan ADDIE (Analysis, Design, Development, Implementation, Evaluation), dan uji efektivitas intervensi. Penelitian telah dilakukan di Kota Banda Aceh dan Aceh Besar pada Januari - April 2025. Sampel terdiri dari 46 ahli gizi (FGD) untuk dilakukan validasi dan uji kelayakan aplikasi, dan 96 responden (uji efektivitas) berusia 18 tahun keatas, yang diambil secara purposif dan random. Terdapat dua kelompok perlakuan yaitu intervensi konseling gizi dengan pendekatan (TTM) dan Model 5A’s. Data dikumpulkan melalui kuesioner pengetahuan, sikap, tindakan, serta uji fungsionalitas aplikasi. Intervensi konseling gizi kepada dilakukan menggunakan aplikasi ObesiCare berbasis dua model perilaku. Analisis data menggunakan uji Blackbox, Repeated Measures ANOVA, dan Difference-in-Difference (DiD) pada tingkat kemaknaan 95%.

Introduction: The prevalence of obesity in Aceh is rising, exacerbated by limited nutritional literacy and suboptimal traditional counseling methods. Digital-based approaches offer promise for enhancing nutritional assessment and education. The use of digital applications as a medium for nutritional counseling is expected to have a positive impact on obesity prevention and management. Objectives: This study aimed to develop a mobile-based assessment and nutrition needs model through the “ObesiCare” application for obesity prevention using the Transtheoretical Model (TTM) and the 5A’s behavioral approach in Aceh. Methods: This mixed-method study (research and development with a quasi-experimental design) consisted of an exploratory study, the development of the ObesiCare application using the ADDIE approach (Analysis, Design, Development, Implementation, Evaluation), and an intervention effectiveness test. The study was conducted in Banda Aceh City and Aceh Besar from January to April of 2025. The sample comprised 46 nutritionists (FGD) for application validation and feasibility testing and 96 respondents (effectiveness testing) aged 18 years and older, selected purposively and randomly. There were two intervention groups: nutrition counseling using the Transtheoretical Model (TTM) and the 5A’s Model. Data were collected using knowledge, attitude, and practice (KAP) questionnaires and application functionality testing. Nutrition counseling interventions were delivered using the ObesiCare application based on two behavioral models. Data analysis employed Blackbox testing, Repeated Measures ANOVA, and Difference-in-Difference (DiD) at a 95% significance level. Results: The ObesiCare application has been developed demonstrated 81.2% functionality and 87.3% usability rates. Its use significantly improved knowledge (p< 0.001; η²p TTM= 0.330; 5A’s= 0.716), attitudes (η²p TTM= 0.810; 5A’s= 0.807), and behavior (η²p TTM= 0.693; 5A’s= 0.859). The 5A’s model was more effective than the TTM, with a mean score difference of +4.5 points. DiD analysis confirmed a significant intervention effect (interaction coefficient= 13.3; p< 0.001; Adjusted R²= 0.695), indicating that the model explained 69.5% of the variance in healthy eating behavior change for obesity prevention among adults aged over 18 years. Conclusion: The ObesiCare application is an effective digital tool for education and assessment in nutrition counseling. The 5A’s behavioral approach is recommended for broader adoption in primary health services for obesity prevention in the Aceh Province.
