PT. XYZ merupakan salah satu produsen crude palm oil (CPO) di Indonesia yang terletak di Sumatera Utara. PT. XYZ pastinya mengupayakan agar nilai kehilangan minyak (oil losses) terjadi seminimal mungkin selama proses produksi. Pengendalian kualitas secara statistik dapat dilakukan menggunakan alat bantu yang terdapat pada statistical process control (SPC). Akar penyebab kegagalan dalam proses produksi juga dapat diketahui dengan menggunakan metode failure mode and effect analysis (FMEA) dan fault tree analysis (FTA). PT. XYZ memiliki norma oil losses pada CPO yaitu tandan kosong sebesar 0,44%, ampas press maksimal 0,52%, kernel 0,10% dan fat fit sebesar 0,35%. Penelitian ini bertujuan untuk menganalisis nilai oil losses PT. XYZ periode bulan September-Desember 2023 dan faktor-faktor apa yang menyebabkan oil losses dengan menggunakan metode SPC, FMEA dan FTA. Hasil penelitian ini menunjukkan nilai oil losses CPO di PT. XYZ belum terkendali secara statistik pada semua sampel oil losses. Sehingga dapat dikatakan bahwa proses yang menyebabkan terjadinya oil losses di PT. XYZ juga belum terkendali secara statistik. Adapun 2 penyebab utama terjadinya oil losses di PT. XYZ yaitu TBS yang terlalu matang (E2) dan tekanan screw press terlalu rendah (E8). Berdasarkan metode FTA, penyebab terjadinya E2 yaitu perencanaan tenaga kerja yang buruk, pekerja kurang displin dan kurangnya pelatihan terhadap pekerja dan penyebab terjadinya E8 yaitu pekerja kurang displin, kurangnya pelatihan terhadap pekerja dan kurangnya preventive maintance terhadap mesin screw press. Rekomendasi perbaikan adalah pemeriksaan terhadap bahan baku, maintance terhadap alat thresher dan screw press secara berkala jika perlu dilakukan perbaikan segera, analisa bahan baku secara konsisten dan pengawasan terhadap operator yang sedang bekerja.

PT. XYZ is one of the producers of crude palm oil (CPO) in Indonesia, located in North Sumatra. PT. XYZ certainly strives to minimize oil losses as possible during the production process. Statistical quality control can be carried out using tools contained in statistical process control (SPC). The root cause of failure in the production process can also be identified using the failure mode and effect analysis (FMEA) and fault tree analysis (FTA) methods. PT. XYZ has oil losses norms for CPO, namely empty bunches of 0.44%, press dregs maximum of 0.52%, kernels of 0.10% and fat fit of 0.35%. This research aims to analyze the value of PT's oil losses. XYZ for the period September-December 2023 and what factors cause oil losses using the SPC, FMEA and FTA methods. The results of this research show the value of CPO oil losses at PT. XYZ has not been statistically controlled in all oil losses samples. So it can be said that the process that causes oil losses at PT. XYZ is also not statistically controlled. There are 2 main causes of oil losses at PT. XYZ is FFB that is too ripe (E2) and the screw press pressure is too low (E8). Based on the FTA method, the causes of E2 are poor workforce planning, workers lack discipline and lack of training for workers and the causes of E8 are workers lack of discipline, lack of training for workers and lack of preventive maintenance of screw press machines. Recommendations for improvement are inspection of raw materials, maintenance of thresher and screw press equipment periodically if repairs are needed immediately, consistent analysis of raw materials and supervision of operators who are working.
