Latar Belakang: Rinosinusitis kronik (RSK) adalah inflamasi persisten mukosa
nasal dan sinus paranasal. Variasi anatomi nasal dan sinus paranasal dapat
mengobstruksi dan mengganggu fungsi drainase dan ventilasi dari ostium sinus
sehingga menyebabkan RSK. Computerized tomography (CT) Scan merupakan
pemeriksaan radiologis dalam penegakan diagnosis RSK dan juga menilai struktur
anatomi nasal dan sinus paranasal. Tujuan: mengetahui hubungan variasi anatomi
nasal dan sinus paranasal dengan derajat keparahan RSK berdasarkan pemeriksaan
CT Scan di RSUDZA Banda Aceh Metode: Penelitian observasional analitik
dengan desain cross sectional dari hasil pemeriksaan CT Scan pasien RSK dalam
periode Oktober 2019-Oktober 2022. Variasi anatomi yang dinilai adalah derajat
deviasi septum nasi, tipe konka bulosa, konka media paradoksal, hipertrofi bulla
ethmoid, sel Haller, defleksi prosessus unsinatus, tipe insersi superior prosessus
unsinatus, pneumatisasi prosessus unsinatus dan sel-sel sinus frontalis terhadap
derajat RSK menggunakan skor Lund Mackay. Data dianalisa menggunakan ChiSquare test atau Fisher’s exact test. Hasil: Analisa dari 64 CT Scan yang terdiri
dari 128 sisi hidung didapatkan hubungan yang signifikan antara variasi anatomi
dengan derajat RSK yaitu derajat deviasi septum nasi, tipe konka bulosa, hipertrofi
bulla ethmoid dan defleksi prosessus unsinatus terhadap derajat keparahan sinusitis
maksilaris (p=0.023; p=0.024; p=0.039; p=0.029) dan konka media paradoksal
terhadap derajat keparahan sinusitis maksilaris, ethmoidalis anterior dan frontalis
(p=0.018; p=0.013; p=0.000) Kesimpulan: Variasi anatomi derajat deviasi septum
nasi, konka bulosa, konka media paradoksal, hipertrofi bulla ethmoid dan defleksi
prosessus unsinatus berhubungan dengan derajat keparahan RSK.

Background: Chronic rhinosinusitis (CRS) is a persistent inflammation of the nasal mucosa and paranasal sinuses. Anatomical variations of the nasal and paranasal sinuses can obstruct and impair the drainage and ventilation functions of the sinus ostium, causing CRS. Computerized tomography (CT) Scan is a radiological examination in diagnosing CRS and also assessing the anatomical structures of the nose and paranasal sinuses. Objective: to determine the relationship between nasal and paranasal sinus anatomical variations with the degree of severity of CRS based on a CT scan at RSUDZA Banda Aceh. Methods: An analytic observational study with a cross-sectional design from the results of CT scans of CRS patients in the period October 2019-October 2022. The anatomical variations assessed were the degree of deviation of the nasal septum, type of bullous turbinate, paradoxical turbinate, hypertrophy of ethmoid bullae, Haller cells, deflection of uncinate process, type of superior insertion of the uncinate process, pneumatization of the uncinate process and frontal sinus cells to the degree of CRS using the Lund Mackay score. Data were analyzed using the Chi-Square test or Fisher's exact test.Result: Analysis of 64 CT scans consisting of 128 sides of the nose found a significant relationship between anatomic variations and the degree of CRS, namely the degree of deviation of nasal septum, type of concha bullosa, ethmoid bulla hypertrophy and deflection of the uncinate process to the severity of maxillary sinusitis (p=0.023; p=0.024; p=0.039; p=0.029) and paradoxical medial concha for the severity of maxillary sinusitis, anterior and frontal ethmoiditis (p=0.018; p=0.013; p=0.000) Conclusion: Anatomical variations in the degree of deviation of the nasal septum, bullous turbinates, paradoxical medial concha, hypertrophy of the ethmoid bulla and deflection of the uncinate process are related to the severity of CRS.
