Penelitian ini bertujuan untuk mengidentifikasi Drug Related Problems (DRPs) dan
menganalisis faktor-faktor yang berhubungan dengan kejadian DRPs pada pasien
gangguan tiroid di RSUD dr. Zainoel Abidin Banda Aceh. Penelitian ini merupakan
penelitian observasional dengan desain cross-sectional yang dilakukan secara
retrospektif terhadap 70 pasien gangguan tiroid yang memenuhi kriteria inklusi.
Identifikasi DRPs dilakukan menggunakan klasifikasi PCNE V9.1, kepatuhan pasien
dinilai menggunakan kuesioner MMAS-8, dan ADR dinilai menggunakan Algoritma
Naranjo. Analisis data meliputi analisis univariat, bivariat menggunakan uji Chi
square, dan multivariat menggunakan regresi logistik. Hasil penelitian menunjukkan
bahwa sebanyak 58 pasien mengalami DRPs. Masalah DRPs yang paling banyak
ditemukan adalah kemungkinan terjadinya efek samping obat dan efek terapi yang
tidak optimal, sedangkan penyebab DRPs yang paling dominan adalah ketidakpatuhan
pasien dalam penggunaan obat. Analisis bivariat menunjukkan bahwa kepatuhan dan
kadar TSH yang berhubungan signifikan dengan kejadian DRPs (p0,05). Analisis multivariat menunjukkan
bahwa kepatuhan merupakan faktor yang paling berhubungan dengan kejadian DRPs
(OR=0,038; 95%CI=0,007–0,217; p

This study aimed to identify Drug-Related Problems (DRPs) and analyze the factors associated with the occurrence of DRPs among patients with thyroid disorders at Dr. Zainoel Abidin Regional General Hospital, Banda Aceh. This observational study employed a retrospective cross-sectional design involving 70 patients with thyroid disorders who met the inclusion criteria. DRPs were identified using the Pharmaceutical Care Network Europe (PCNE) Classification V9.1, patient adherence was assessed using the 8-item Morisky Medication Adherence Scale (MMAS-8), and adverse drug reactions (ADRs) were evaluated using the Naranjo Algorithm. Data were analyzed using univariate analysis, bivariate analysis with the Chi-square test, and multivariate analysis using logistic regression. The results showed that 58 patients experienced DRPs. The most frequently identified DRP problems were the potential occurrence of adverse drug reactions and suboptimal therapeutic effects, whereas the most common cause of DRPs was patient non-adherence to medication. Bivariate analysis demonstrated that medication adherence and thyroid-stimulating hormone (TSH) levels were significantly associated with the occurrence of DRPs (p < 0.001), whereas age, sex, comorbidities, polypharmacy, TSH levels, and drug dosage showed no significant association (p > 0.05). Multivariate analysis revealed that medication adherence was the factor most strongly associated with DRPs (OR = 0.038; 95% CI = 0.007–0.217; p < 0.001). In conclusion, the incidence of DRPs among patients with thyroid disorders at Dr. Zainoel Abidin Regional General Hospital remains high, with patient non-adherence identified as the predominant contributing factor. Therefore, improving patient education and adherence monitoring is essential to minimize the occurrence of DRPs and enhance therapeutic outcomes.
