Spastisitas merupakan komplikasi yang sering terjadi pada pasien stroke iskemik dan menyebabkan peningkatan tonus otot serta penurunan kemampuan fungsional. Penelitian ini bertujuan menganalisis pengaruh edukasi latihan passive stretching berbasis primary caregiver terhadap derajat spastisitas pada pasien stroke iskemik di Rumah Sakit Umum Daerah dr. Zainoel Abidin Banda Aceh. Penelitian ini menggunakan desain quasi-experimental. Sampel sebanyak 70 pasien dipilih dengan teknik non-probability sampling metode consecutive sampling dan dibagi menjadi kelompok intervensi (n=35) serta kontrol (n=35). Kelompok intervensi memperoleh edukasi dan latihan passive stretching tiga kali sehari selama 20 hari, sedangkan kelompok kontrol memperoleh routine care. Spastisitas diukur menggunakan Modified Ashworth Scale (MAS). Analisis menggunakan uji Wilcoxon Signed-Rank dan Mann Whitney U - test. Spasticity is a common complication in patients with ischemic stroke and results in increased muscle tone and reduced functional ability. This study aimed to analyze the effect of primary caregiver-based passive stretching education on the degree of spasticity in patients with ischemic stroke at dr. Zainoel Abidin Regional General Hospital, Banda Aceh. This study used a quasi-experimental design. A total of 70 patients were selected using a non-probability sampling technique with the consecutive sampling method and were assigned to an intervention group (n = 35) and a control group (n = 35). The intervention group received education and passive stretching exercises three times daily for 20 days, whereas the control group received routine care. Spasticity was measured using the Modified Ashworth Scale (MAS). Data were analyzed using the Wilcoxon Signed-Rank Test and the Mann–Whitney U test. The intervention group showed a significant reduction in the degree of spasticity (z = -5.512; p < 0.001), whereas the control group showed no significant change (z = -1.355; p = 0.175). The post-test analysis showed a significant difference in the degree of spasticity between the intervention and control groups (z = -5.656; p < 0.001). Primary caregiver-based passive stretching education was effective in reducing the degree of spasticity in patients with ischemic stroke. This intervention is recommended as part of nursing education provided by nurses through discharge planning for primary caregivers to ensure continuity of patient rehabilitation at home.
