Apendisitis akut merupakan kasus gawat darurat abdomen tersering yang dapat
menimbulkan komplikasi bila penanganannya terlambat. Hasil pemeriksaan darah
seperti hitung jenis leukosit dapat digunakan sebagai indikator inflamasi sistemik,
dengan Systemic Immune-Inflammation Index (SII) dan Systemic Inflammation
Response Index (SIRI), yang berpotensi membantu menilai tingkat keparahan
apendisitis secara cepat. Penelitian ini bertujuan untuk menganalisis perbedaan nilai
indeks SII dan SIRI antara pasien apendisitis akut komplikata dan non komplikata,
serta menilai nilai diagnostik kedua indeks tersebut meliputi sensitivitas,
spesifisitas, cut-off, dan AUC dalam membedakan kedua kelompok. Penelitian ini
merupakan studi observasional analitik dengan desain cross-sectional retrospektif
menggunakan data rekam medis pasien apendisitis akut yang menjalani operasi di
RSUD Zainoel Abidin Banda Aceh periode Januari 2023–Desember 2024.
Penelitian mengumpulkan 77 data rekam medis dari pasien Apendisitis akut yang
menjalani operasi apendektomi sejak 1 Januari 2023 hingga 31 Desember 2024.
Hasil penelitian menunjukkan bahwa terdapat perbedaan signifikan antara indeks
SII dan SIRI pada pasien apendisitis akut komplikata dan non komplikata (P-value

Acute appendicitis is one of the most common abdominal emergencies and may lead to complications if treatment is delayed. Blood test parameters, such as leukocyte differential count, can be utilized as indicators of systemic inflammation, including the Systemic Immune-Inflammation Index (SII) and the Systemic Inflammation Response Index (SIRI), which have the potential to assist in rapidly assessing the severity of appendicitis. This study aimed to analyze the differences in SII and SIRI values between patients with complicated and non-complicated acute appendicitis, and to evaluate the diagnostic performance of both indices, including sensitivity, specificity, cut-off values, and area under the curve (AUC), in distinguishing the two groups. This research was an analytical observational study with a retrospective cross-sectional design using medical record data of patients with acute appendicitis who underwent appendectomy at Zainoel Abidin General Hospital, Banda Aceh, from January 2023 to December 2024. A total of 77 medical records were included. The results showed significant differences in SII and SIRI values between patients with complicated and non-complicated acute appendicitis (p-value < 0.001 and 0.005, respectively). ROC curve analysis demonstrated that an SII cut-off value of 1130.25 yielded an AUC of 0.78, with a sensitivity of 92.10% and a specificity of 51%. Meanwhile, a SIRI cut-off value of 31.9 produced an AUC of 0.69, with a sensitivity of 86.80% and a specificity of 46.20%. In conclusion, both indices exhibited adequate diagnostic performance and may serve as simple hematological parameters to aid in the early detection of complicated acute appendicitis, particularly in primary healthcare settings.
