Pendahuluan. Levothyroxine sering kali diberikan pada pasien dengan hipotiroid, namun menentukan waktu optimal untuk deprescribing levothyroxine dan mengidentifikasi tipe pasien tertentu yang akan mendapatkan manfaat paling besar masih menjadi bahan perdebatan. Dalam praktik klinis, deprescribing levothyroxine sangat penting untuk mengurangi risiko pengobatan yang berlebihan, karena banyak pasien yang sedang mengonsumsi obat mungkin tidak memperoleh manfaat yang signifikan dan berpotensi mengalami bahaya. Berbagai penelitian telah meneliti waktu tindak lanjut setelah meresepkan levothyroxine. Di sini, penelitian ini bertujuan untuk membandingkan efektivitas 6 minggu dan 12 minggu dalam menentukan waktu optimal deprescribing levothyroxine dan mengukur penanda prediktif untuk mengidentifikasi kandidat ideal untuk deprescribing levothyroxine.
Tujuan. Untuk mengetahui tingkat keberhasilan deprescribing levothyroxine selama 6 minggu dan 12 minggu pada penderita hipotiroid.
Metode. Studi kohort prospektif dilakukan di rumah sakit rujukan tersier Dr. Rumah Sakit Zainoel Abidin di Banda Aceh, Indonesia dari 12 September 2023 hingga 12 September 2024. Pasien yang terdaftar dipilih dengan pengambilan sampel berturut-turut dan menandatangani formulir persetujuan. Skrining kelayakan pasien dilakukan di Pusat Tiroid RS Dr. Zainoel Abidin. Levothyroxine dihentikan sepenuhnya bila hasil laboratorium pasien menunjukkan kadar tiroksin bebas (FT4), dan penekanan hormon perangsang tiroid (TSH) yang tidak lengkap selama lebih dari 6 bulan. Hasil dinilai pada awal deprescribing levothyroxine, setelah 6 dan 12 minggu. Data berikut didokumentasikan: jenis kelamin, usia, diagnosis hipotiroid, durasi terapi levothyroxine, indeks massa tubuh (BMI), serum TSH, serum FT4, profil lipid.
Hasil. Pada penelitian ini didapatkan bahwa 19 orang yang menjalani deprescribing terdapat 14 orang (73,69%) tetap mengalami eutiroid, dan sisanya mengalami peningkatan TSHs kembali. Rata-rata TSHs pada penelitian ini saat di lakukan deprescribing sebesar 2.85 ± 1.82 μIU/L. Dalam penelitia ini, terdapat perbedaan segnifikan antara nilai TSHs awal dengan minggu ke-6 dengan P 0,005.
Kesimpulan. Minggu ke-6 merupakan waktu yang cukup untuk evaluasi TSHs pada deprescribing levothyroxine.

ABSTRACT Introduction. Levothyroxine is frequently prescribed to patients with hypothyroidism, but determining the optimal timing for deprescribing levothyroxine and identifying the specific types of patients who will derive the greatest benefit remains a matter of debate. In clinical practice, deprescribing levothyroxine is essential to reduce the risk of overtreatment, as many patients currently taking the drug may not obtain significant benefit and may potentially experience harm. Various studies have examined follow-up time after deprescribing levothyroxine. Here, this study aimed to compare the effectiveness of 6 weeks and 12 weeks in determining the optimal timing of deprescribing levothyroxine and measure predictive markers to identify ideal candidates for deprescribing levothyroxine. Objective. To determine the success rate of deprescribing levothyroxine for 6 weeks and 12 weeks in hypothyroid sufferers. Method. A prospective cohort study was conducted at the tertiary referral hospital Dr. Zainoel Abidin Hospital in Banda Aceh, Indonesia from 12 September 2023 to 12 September 2024. Registered patients were selected by consecutive sampling and signing a consent form. Patient eligibility screening is carried out at the Thyroid Center of Dr. Hospital. Zainoel Abidin. Levothyroxine is completely discontinued if the patient's laboratory results show free thyroxine (FT4) levels, and incomplete thyroid-stimulating hormone (TSH) suppression for more than 6 months. Outcomes were assessed at the start of levothyroxine deprescribing, after 6 and 12 weeks. The following data were documented: gender, age, diagnosis of hypothyroidism, duration of levothyroxine therapy, body mass index (BMI), serum TSH, serum FT4, lipid profile. Results. In this study, it was found that of the 19 people who underwent deprescribing, 14 people (73.69%) still experienced euthyroidism, and the rest experienced an increase in TSHs again. The average TSHs in this study when deprescribing was carried out was 2.85 ± 1.82 μIU/L. In this study, there was a significant difference between TSH values at baseline and week 6 with P 0.005. Conclusion. Week 6 is sufficient time to evaluate TSHs when deprescribing levothyroxine. Key words: Hypothyroidism, deprescribing, levothyroxine, TSHs, lipid profile
