Buku KIA merupakan alat skrining dini untuk menjaga kesehatan ibu, bayi, dan balita. Pengisian buku kesehatan ibu dan anak yang tidak lengkap akan memberikan dampak tidak terpantau dan tercatatnya kesehatan ibu dan anak sejak hamil. Tujuan penelitian ini adalah untuk mengetahui kelengkapan pengisian buku KIA pada ibu hamil di Rumah Sakit Banda Aceh. Penelitian ini merupakan penelitian deskriptif. Teknik sampling menggunakan total sampling yang berjumlah 49 orang ibu hamil trimester III. Hasil penelitian menunjukkan bahwa dari pengisian item identitas ibu didapatkan item yang tidak lengkap diisi yaitu foto sebesar 0%, No.JKN/Faskes TK 1/Faskes Rujukan sebesar 46,9%, golongan darah sebesar 55,1%, telepon sebesar 63,3%, dan item puskesmas domisili /No.register kohort ibu sebesar 26,5%. Pelayanan kesehatan yang sudah diterima ibu di trimester I, II, dan III yang selalu tidak lengkap diisi yaitu PPIA dan tata laksana kasus. Pengawasan minum tablet tambah darah yang paling sedikit diisi yaitu bulan ke-8 sebesar 4,1%, dan tidak diisi pada bulan ke-9 sebesar 0%. Amanat persalinan tidak terisi lengkap 0%. Pelayanan dokter yang paling sedikit diisi yaitu hasil USG trimester I sebesar 10,2%, konseling sebesar 38,8%, dan hasil USG trimester III sebesar 20,4%. Dapat disimpulkan bahwa mayoritas buku KIA yang didapatkan tidak ada item yang lengkap diisi oleh tenaga kesehatan.

The MCH book is an early screening tool to maintain the health of mothers, infants, and toddlers. Incomplete filling of maternal and child health books will have an impact on not monitoring and recording the health of mothers and children since pregnancy. The purpose of this study was to determine the completeness of MCH book filling in pregnant women at Banda Aceh Hospital. This study is a descriptive study. The sampling technique used total sampling which amounted to 49 third trimester pregnant women. The results showed that from the filling of maternal identity items, incomplete items were obtained, namely photos by 0%, No.JKN / TK 1 / Referral Facility by 46.9%, blood type by 55.1%, telephone by 63.3%, and the item domicile health center / No.register of maternal cohort by 26.5%. Health services received by mothers in the first, second, and third trimesters that were always incompletely filled in were PPIA and case management. The least completed supervision of taking blood supplement tablets was the 8th month at 4.1%, and was not completed in the 9th month at 0%. The mandate for delivery was incompletely filled in 0%. The least filled doctor's services were ultrasound results of the first trimester by 10.2%, counseling by 38.8%, and ultrasound results of the third trimester by 20.4%. It can be concluded that the majority of MCH book obtained had no items that were completely filled in by health workers. Keywords: MCH book, pregnant women, third trimester
