Abstrak latar belakang: polisomnografi (psg) merupakan baku emas diagnosis osa, namun keterbatasan ketersediaan, biaya, dan sumber daya menjadikan pemeriksaan ini sulit diakses secara luas. paediatric sleep questionnaire (psq) merupakan instrumen diagnosis awal non-invasif yang berpotensi menjadi alternatif awal dalam mendeteksi osa pada anak. obstructive sleep apnea (osa) pada anak merupakan gangguan pernapasan saat tidur yang dapat menimbulkan dampak signifikan terhadap fungsi neurokognitif, kardiovaskular, dan pertumbuhan. tujuan: menilai akurasi psq terhadap psg, apakah dapat sebagai alat diagnostik diagnosis awal mengidentifikasi anak yang berisiko terkena osa di rsud dr. zainoel abidin banda aceh. metode: penelitian ini menggunakan desain analitik diagnostik dengan pendekatan potong lintang. subjek penelitian adalah anak yang dicurigai mengalami osa berdasarkan anamnesis dan pemeriksaan klinis. seluruh subjek menjalani pengisian psq (22 item) dan pemeriksaan psg sebagai baku emas. hasil: skor psq ≥ 0,33 dijumpai pada 12 anak, yaitu 9 anak (45%) osa ringan, 3 anak (15%) osa sedang, dan tidak dijumpai osa berat. didapatkan prevalensi osa sebesar 60%. mayoritas subjek penelitian berusia 11-19 tahun (50%), dengan rentang usia termuda 1 tahun dan tertua 18 tahun. rata-rata usia responden adalah 10,6 ± 5,7. terdapat 10 (50%) laki-laki dengan usia rata-rata 11 ± 4,80 (sd) dan 10 (50%) perempuan dengan usia rata-rata 10,2 ± 6,82 (sd). untuk status gizi cukup bervariasi yaitu 45% gizi baik, 20% malnutrisi (gizi buruk dan kurang), dan 35% gizi lebih hingga obesitas. subjek penelitian yang dominan mengalami osa pada kelompok gizi baik; 4 subjek osa ringan (20%) dan 1 subjek osa sedang (5%). pada kelompok gizi kurang; 1 subjek osa ringan, dan pada kelompok gizi lebih; masing-masing 1 subjek osa ringan dan osa sedang. pada 3 anak obesitas mengalami osa ringan (15%) dan 1 anak dengan osa sedang. tidak dijumpai osa berat pada semua status gizi. untuk faktor risiko hipertrofi adenoid/tonsil merupakan yang terbanyak; 6 anak osa ringan (30%) dan 2 anak osa sedang (10%). faktor risiko obesitas pada 3 anak dengan osa ringan (15%) dan 1 anak dengan osa sedang (5%). sedangkan 2 anak dengan disproporsi kraniofasial tidak dengan osa. osa berat tidak ditemukan pada ketiga faktor risiko tersebut. skor psq menunjukkan hubungan yang sangat bermakna dengan derajat osa. kasus osa ringan (45%) dan sedang (15%) ditemukan pada hampir seluruh subjek dengan skor psq ≥ 0,33 dan tidak terdapat kasus osa berat. variabel jenis kelamin, usia, status gizi, dan faktor risiko tidak menunjukkan hubungan yang bermakna secara statistik dengan derajat osa yang dinilai dari pemeriksaan psg. namun, skor psq menunjukkan korelasi yang kuat secara statistik terhadap tingkat keparahan osa. kesimpulan: paediatric sleep questionnaire memiliki akurasi yang baik sebagai alat diagnosis awal untuk obstructive sleep apnea pada anak. psq dapat digunakan secara luas sebagai instrumen diagnostik sederhana, non-invasif, dan hemat biaya, terutama di fasilitas pelayanan kesehatan yang belum memiliki sarana polisomnografi, guna mendukung deteksi dini dan tatalaksana osa pada anak. kata kunci: obstructive sleep apnea pada anak, paediatric sleep questionnaire, polisomnografi.

Abstract
ABSTRACT Background: Polysomnography (PSG) is the gold standard for diagnosing OSA, but limitations in availability, cost, and resources make this test difficult to access widely. The Paediatric Sleep Questionnaire (PSQ) is a non-invasive screening tool that has the potential to serve as an initial alternative for detecting OSA in children. Obstructive Sleep Apnea (OSA) in children is a sleep-related breathing disorder that can have significant impacts on neurocognitive function, cardiovascular health, and growth. Objective: To assess the accuracy of the Pediatric Sleep Questionnaire compared to polysomnography, and whether it can be used as a first-line diagnostic screening tool to identify children at risk of obstructive sleep apnea at Dr. Zainoel Abidin Regional General Hospital in Banda Aceh. Methods: This study used a diagnostic analytical design with a cross-sectional approach. The research subjects were children suspected of having OSA based on medical history and clinical examination. All subjects completed the Paediatric Sleep Questionnaire (22 items) and underwent polysomnography as the gold standard Results: PSQ scores ≥ 0.33 were found in 12 children, namely 9 children (45%) with mild OSA, 3 children (15%) with moderate OSA, and no children with severe OSA. The prevalence of OSA was 60%. The majority of study subjects were aged 11-19 years (50%), with the youngest being 1 year old and the oldest being 18 years old. The mean age of respondents was 10.6 ± 5.7. There were 10 (50%) males with an average age of 11 ± 4.80 (SD) and 10 (50%) females with an average age of 10.2 ± 6.82 (SD). Nutritional status varied, with 45% having good nutrition, 20% malnutrition (poor and deficient nutrition), and 35% overnutrition to obesity. The majority of research subjects experienced OSA in the good nutrition group; 4 subjects had mild OSA (20%) and 1 subject had moderate OSA (5%). In the underweight group, there was 1 subject with mild OSA, and in the overweight group, 1 subject each with mild OSA and moderate OSA. Three obese children experienced mild OSA (15%) and 1 child with moderate OSA. No severe OSA was found in any nutritional status group. The most common risk factor was adenoid/tonsil hypertrophy; 6 children with mild OSA (30%) and 2 children with moderate OSA (10%). The risk factor of obesity was found in 3 children with mild OSA (15%) and 1 child with moderate OSA (5%). Meanwhile, 2 children with craniofacial disproportion did not have OSA. Severe OSA was not found in any of the three risk factors. The Pediatric Sleep Questionnaire (PSQ) score showed a highly significant relationship with the degree of OSA. Mild (45%) and moderate (15%) OSA cases were found in almost all subjects with a PSQ score ≥ 0.33, and there were no cases of severe OSA. The variables of gender, age, nutritional status, and risk factors did not show a statistically significant relationship with the degree of OSA as assessed by polysomnography (PSG). However, the Pediatric Sleep Questionnaire (PSQ) score showed a strong statistical correlation with the severity of Obstructive Sleep Apnea (OSA). Conclusion: The Pediatric Sleep Questionnaire has adequate accuracy as an initial screening tool for obstructive sleep apnea in children.
