Infeksi saluran kemih (isk) merupakan salah satu infeksi nosokomial yang paling sering terjadi, terutama pada pasien dengan penggunaan kateter urin. pasien diabetes melitus (dm) memiliki risiko lebih tinggi mengalami isk serta pembentukan biofilm yang dapat meningkatkan resistensi antibiotik. penelitian ini bertujuan untuk mengetahui hubungan pembentukan biofilm pada spesimen urin kateter pasien isk dengan dm dan tanpa dm serta hubungannya dengan lama penggunaan kateter. penelitian ini menggunakan desain kasus kontrol dengan pendekatan potong lintang yang melibatkan 78 pasien isk pengguna kateter, terdiri dari 39 pasien dm dan 39 pasien tanpa dm. spesimen urin kateter dikultur, diidentifikasi bakterinya, dan diuji pembentukan biofilm menggunakan metode microtiter plate assay (mpa). analisis data dilakukan secara univariat dan bivariat menggunakan uji korelasi pearson. hasil penelitian menunjukkan bahwa sebagian besar isolat bakteri memiliki kemampuan membentuk biofilm kategori kuat, baik pada kelompok dm (64,1%) maupun tanpa dm (71,8%). bakteri penyebab isk didominasi oleh bakteri gram negatif, terutama escherichia coli, dan klebsiella pneumoniae, sedangkan dari kelompok gram positif yang paling sering ditemukan adalah enterococcus faecalis. rerata nilai optical density (od) pada kelompok dm adalah 0,489 ± 0,341 dan pada kelompok tanpa dm adalah 0,847 ± 0,693. terdapat korelasi positif antara lama penggunaan kateter dan pembentukan biofilm pada seluruh subjek (r = 0,263; p = 0,020). pada kelompok tanpa dm, hubungan ini signifikan (r = 0,338; p = 0,036), sedangkan pada kelompok dm tidak signifikan (r = 0,155; p = 0,345). kesimpulan penelitian ini adalah sebagian besar bakteri penyebab isk memiliki kemampuan kuat dalam membentuk biofilm pada kateter urin. lama penggunaan kateter berhubungan dengan peningkatan pembentukan biofilm, terutama pada pasien tanpa dm. oleh karena itu, penggunaan kateter urin sebaiknya dibatasi seminimal mungkin untuk mengurangi risiko infeksi dan pembentukan biofilm. kata kunci: biofilm, ca-uti, diabetes melitus, infeksi saluran kemih, kateter urin



Abstract

Urinary tract infection (UTI) is one of the most common nosocomial infections, particularly in patients with indwelling urinary catheters. Patients with diabetes mellitus (DM) have a higher risk of developing UTIs and biofilm formation, which contributes to antibiotic resistance. This study aimed to determine the relationship between biofilm formation in catheter urine specimens of UTI patients with and without DM, as well as its association with catheterization duration. This was a case-control study with a cross-sectional design involving 78 catheterized UTI patients, including 39 with DM and 39 without DM. Catheter urine specimens were cultured, bacterial identification was performed, and biofilm formation was assessed using the Microtiter Plate Assay (MPA). Data were analyzed using univariate and bivariate methods with Pearson correlation tests. The results demonstrated that the majority of bacterial isolates exhibited strong biofilmforming capability in both DM (64.1%) and non-DM (71.8%) groups. UTI pathogens were predominantly Gram-negative bacteria, particularly Escherichia coli and Klebsiella pneumoniae, while the most frequently identified Gram-positive bacteria was Enterococcus faecalis. The mean optical density (OD) was 0.489 ± 0.341 in the DM group and 0.847 ± 0.693 in the non-DM group. A positive correlation was found between catheterization duration and biofilm formation in all subjects (r = 0.263; p = 0.020). This correlation was statistically significant in the non-DM group (r = 0.338; p = 0.036), whereas no statistically significant correlation was observed in the DM group (r = 0.155; p = 0.345). In conclusion, the majority of UTI-causing bacteria demonstrated a strong capability to form biofilms on urinary catheters. Longer catheterization duration is associated with increased biofilm formation, particularly in non-diabetic patients. Therefore, urinary catheter use should be minimized to reduce the risk of infection and biofilm development.
