Abstrak infeksi helicobacter pylori merupakan penyebab utama ulkus saluran cerna atas. real-time polymerase chain reaction (rt pcr) menjadi alternatif metode diagnostik yang menjanjikan dengan kemampuan mendeteksi dna bakteri dari berbagai jenis sampel, termasuk feses dan saliva. penelitian ini merupakan studi observasional komparatif dengan desain potong lintang pada pasien ulkus saluran cerna atas yang dikonfirmasi melalui esofagogastroduodenoskopi (egd) di rsud dr. zainoel abidin banda aceh. sebanyak 17 sampel feses dan saliva dikumpulkan dan dianalisis menggunakan real-time pcr dengan target gen hp23f untuk mendeteksi h. pylori. tingkat kesesuaian )concordance) hasil antara kedua jenis sampel dievaluasi untuk menilai konsistensi deteksi antar metode. pcr feses mendeteksi h. pylori pada 8 dari 17 sampel (47,1%), sedangkan pcr saliva mendeteksi 16 dari 17 sampel (94,2%). tingkat kesesuaian ( ) antar metode sebesar 58,8%, sedangkan ketidaksesuaian (discordance) sebesar 41,2%; seluruh pasangan discordant menunjukkan pola searah, yaitu hasil positif pada saliva namun negatif pada feses, tanpa satu pola sebaliknya. proporsi deteksi h. pylori pada saliva yang jauh lebih tinggi dibandingkan feses, disertai pola discordance yang konsisten searah, mengindikasikan adanya perbedaan karakteristik biologis antara kedua jenis spesimen dalam mendeteksi keberadaan h. pylori. feses dan saliva menunjukkan tingkat kesesuaian yang sedang sehingga tidak dapat digunakan secara saling menggantikan, dan pemilihan jenis sampel perlu mempertimbangkan konteks biologis serta tujuan klinis pemeriksaan. kata kunci:h. pylori, pcr, stool, saliva, ulkus saluran cerna atas

Abstract
ABSTRACT Helicobacter pylori infection is a leading cause of upper gastrointestinal ulcers. Real-Time Polymerase Chain Reaction (RT-PCR) has emerged as a promising non-invasive diagnostic alternative capable of detecting bacterial DNA from various specimen types, including stool and saliva. This comparative observational study employed a cross-sectional design among patients with upper gastrointestinal ulcers confirmed by esophagogastroduodenoscopy (EGD) at dr. Zainoel Abidin Regional General Hospital, Banda Aceh. A total of 17 paired stool and saliva samples were collected and analyzed using real-time PCR targeting the HP23F gene to detect H. pylori. Stool PCR served as the reference standard, while the concordance between the two specimen types was evaluated to assess the consistency of detection across methods. Stool PCR detected H. pylori in 8 of 17 samples (47.1%), while saliva PCR yielded positive results in 16 of 17 samples (94.2%). The concordance rate between the two methods was 58.8%, while the discordance rate was 41.2%; all discordant pairs followed a unidirectional pattern, with positive saliva results paired with negative stool results, and no pairs showing the reverse pattern. The markedly higher detection rate in saliva compared with stool, together with the consistently unidirectional discordance pattern, suggests underlying biological differences between the two specimen types in detecting H. pylori. Stool and saliva PCR showed only moderate agreement and therefore cannot be used interchangeably; the choice of specimen type should take into account its biological context and the clinical purpose of testing.
