Latar belakang : anak pengidap talasemia membutuhkan transfusi darah berulang yang berisiko mencetuskan kelebihan zat besi (iron overload) sehingga berpotensi memicu kerusakan organ, terutama hati dan ginjal. feritin serum difungsikan selaku indikator beban besi tubuh, dan meningkatnya kadar feritin serum berkaitan dengan terjadinya gangguan fungsi hati serta fungsi ginjal pada anak pengidap talasemia. tujuan : kajian ini ditujukan guna mengetahui keterkaitan kadar feritin serum terhadap fungsi hati dan fungsi ginjal pada pasien anak pengidap talasemia di rsud dr. zainoel abidin banda aceh. metode: studi analitik observasional berdesain cross-sectional diterapkan pada kajian ini terhadap anak berusia 6 bulan hingga 18 tahun terdiagnosis talasemia, diselenggarakan di poliklinik talasemia rsudza dr. zainoel abidin banda aceh sepanjang desember 2025 hingga januari 2026 yang memenuhi kriteria kajian. hasil : sepanjang kajian, 80 pasien anak pengidap talasemia diperoleh, terdiri atas 48 laki-laki (60%) dan 32 perempuan (40%), dengan kelompok usia terbanyak 6– 12 tahun (61,3%), mayoritas berstatus gizi kurang (42,5%), sebagian mengalami hepatomegali (45%) dan splenomegali (48,8%), anemia sedang (68,8%), serta sebagian besar menerima transfusi dua kali per bulan (66,3%). sejumlah 58 anak (72,5%) memiliki kadar feritin >2000 ng/ml dan 22 anak (27,5%) 0,05). analisis multivariat memperlihatkan kadar feritin serum berkaitan dengan peningkatan sgpt selaku indikator gangguan fungsi hati, namun tidak berkaitan bermakna dengan fungsi ginjal. kesimpulan : keterkaitan bermakna dijumpai antara kadar feritin serum dengan peningkatan sgpt selaku indikator gangguan fungsi hati pada pasien anak pengidap talasemia. keterkaitan bermakna antara kadar feritin serum dengan parameter fungsi ginjal tidak dijumpai. kata kunci : feritin serum, fungsi hati, fungsi ginjal, talasemia beta mayor

Abstract
Background: Children with thalassemia necessitate recurrent blood transfusions, elevating the risk of iron overload and potentially causing organ damage, especially to the liver and kidneys. Serum ferritin serves as a marker of the body's iron burden, and heightened serum ferritin levels are linked to compromised hepatic and renal function in thalassemic children. Objective: To evaluate the relationship between serum ferritin levels and hepatic as well as renal function among pediatric thalassemia patients at Dr. Zainoel Abidin Regional General Hospital, Banda Aceh. Methods: An observational analytic investigation utilizing a cross-sectional design was carried out among children aged 6 months to 18 years diagnosed with thalassemia who visited the thalassemia clinic at Dr. Zainoel Abidin Regional General Hospital, Banda Aceh, spanning December 2025 through January 2026 and satisfying the study criteria. Results: Eighty pediatric thalassemia patients were enrolled, comprising 48 males (60%) and 32 females (40%). The predominant age group was 6–12 years (61.3%), with undernutrition noted in 42.5%. Hepatomegaly was identified in 45%, splenomegaly in 48.8%, moderate anemia in 68.8%, and the majority underwent transfusions twice monthly (66.3%). Fifty-eight patients (72.5%) exhibited serum ferritin levels >2000 ng/mL while 22 patients (27.5%) had levels 0.05). Multivariate analysis indicated that serum ferritin levels correlated with elevated SGPT as a marker of hepatic dysfunction, yet showed no significant correlation with renal function. Conclusion: A statistically significant correlation was identified between serum ferritin levels and elevated SGPT as a marker of hepatic impairment in pediatric thalassemia patients. No statistically significant correlation was identified between serum ferritin levels and renal function parameters. Keywords: Serum ferritin, liver function, renal function, beta thalassemia major
