Pendahuluan: gagal jantung merupakan sindrom klinis akibat gangguan struktural atau fungsional jantung yang berdampak pada meningkatnya morbiditas dan mortalitas. inflamasi berperan penting dalam progresivitas penyakit ini, dan beberapa biomarker inflamasi terbukti berhubungan dengan prognosis. rasio neutrofil limfosit (rnl) dan rasio platelet limfosit (rpl) merupakan parameter hematologi sederhana, murah, dan mudah diperoleh dari pemeriksaan darah rutin, sehingga berpotensi digunakan dalam praktik klinis untuk menilai status inflamasi dan kualitas hidup pasien gagal jantung. tujuan: penelitian ini bertujuan mengetahui hubungan rnl dan rpl dengan kualitas hidup pasien gagal jantung di rsud dr. zainoel abidin. metode: penelitian analitik observasional dengan desain cross sectional pada 94 pasien gagal jantung yang dipilih secara purposive sampling. data primer berupa kuesioner kualitas hidup, sedangkan data sekunder dari hasil laboratorium darah pasien. analisis hubungan rnl dan rpl dengan kualitas hidup dilakukan menggunakan uji korelasi rank spearman dengan p < 0,05. hasil: dari 94 pasien, 72,3% laki-laki dan 27,7% perempuan, dengan usia terbanyak 61–70 tahun (46,8%). uji korelasi menunjukkan hubungan bermakna antara rnl dengan kualitas hidup (r=0,249; p=0,015) serta rpl dengan kualitas hidup (r=0,269; p=0,009). semakin tinggi rnl dan rpl, semakin rendah kualitas hidup pasien gagal jantung, namun tingkat kekuatan hubungan antara variabel lemah. kesimpulan: penelitian ini mendapatkan adanya hubungan antara rnl dan rpl dengan kualitas hidup pada pasien gagal jantung di rsudza. kata kunci: gagal jantung, rasio neutrofil limfosit, rasio platelet limfosit, kualitas hidup



Abstract

Introduction: Heart failure is a clinical syndrome caused by structural or functional cardiac abnormalities, leading to increased morbidity and mortality. Inflammation plays an important role in disease progression, and several inflammatory biomarkers have been shown to be associated with prognosis. The neutrophil-to-lymphocyte ratio (NLR) and platelet-to-lymphocyte ratio (PLR) are simple, inexpensive hematological parameters that can be obtained from routine blood tests, and therefore have potential use in clinical practice to assess inflammatory status and quality of life in heart failure patients. Objective: This study aimed to determine the association between NLR and PLR with quality of life in heart failure patients at dr. Zainoel Abidin General Hospital. Methods: This observational analytic study with a cross-sectional design included 94 heart failure patients selected by purposive sampling. Primary data were obtained using a quality-of-life questionnaire, while secondary data were collected from laboratory records to calculate NLR and PLR. The relationship between NLR, PLR, and quality of life was analyzed using Spearman’s rank correlation test, with p < 0.05 considered statistically significant. Results: Among the 94 patients, 72.3% were male and 27.7% were female, with the majority aged 61–70 years (46.8%). Correlation analysis showed a significant association between NLR and quality of life (r=0.249; p=0.015), as well as between PLR and quality of life (r=0.269; p=0.009). Higher NLR and PLR values were associated with poorer quality of life in heart failure patients,but the level of strength of the relationship between the variable is weak. Conclusion: This study found relationship between NLR and PLR with quality of life in heart failure patients at Dr. Zainoel Abidin General Hospital.
