Abstrak latar belakang. sepsis merupakan kondisi kegagalan organ yang mengancam jiwa akibat respon imun tubuh yang tidak teratur terhadap infeksi. dalam praktik klinis, skor pelod-2 umum digunakan untuk mengevaluasi keterlibatan berbagai organ dan memperkirakan tingkat mortalitas pada populasi anak. salah satu komponen dari skor ini adalah jumlah trombosit. parameter seperti mpv dan pdw termasuk dalam indikator trombosit yang berpotensi memberikan gambaran prognostik terhadap pasien dengan sepsis apabila diperiksa pada hari ke-3. tujuan. memahami korelasi antara nilai mpv dan pdw terhadap skor pelod-2 pada anak dengan sepsis. metode. penelitian ini merupakan studi kohort prospektif yang melibatkan pasien sepsis berusia di bawah 18 tahun yang dirawat di picu rsud dr. zainoel abidin selama periode juli hingga september 2024. penilaian terhadap skor pelod-2 serta pemeriksaan darah rutin dilakukan pada hari pertama dan hari ketiga perawatan. analisis hubungan antar variabel dilakukan menggunakan uji korelasi pearson. hasil. diperoleh total 44 subjek, kelompok usia terbanyak 10,4 fl sebesar 68,2%. nilai pdw hari ke-1 dan pdw hari ke-3 terbanyak pada batas normal (10 – 18 fl) yaitu 59,1 % dan 61,4 %. ada korelasi sedang antara nilai mpvhari ke-3 dan skor pelod-2 (p = 0,06 ; r = 0,410), korelasi sedang antara pdw hari ke-3 dan skor pelod-2 (p = 0,001 ; r = 0,518), serta korelasi sedang antara perubahan nilai pdw dan perubahan skor pelod-2 (p = 0,005 dan r = 0,471). kesimpulan. kenaikan nilai mpv dan pdw serta perubahan nilai pdw berkorelasi terhadap skor pelod-2 pada anak dengan sepsis. kata kunci: mean platelet volume, platelet distribution width, pelod-2, sepsis, anak

Abstract
ABTRACT Background. Sepsis is a life-threatening condition characterized by organ dysfunction due to a dysregulated immune response to infection. In clinical practice, the PELOD-2 score is commonly used to assess multi-organ involvement and estimate mortality risk in the pediatric population. One of the components of this score is the platelet count. Parameters such as Mean Platelet Volume (MPV) and Platelet Distribution Width (PDW) are among the platelet indices that may offer prognostic insight in patients with sepsis when measured on the third day of treatment. Objective. To evaluate the correlation between MPV and PDW values with the PELOD-2 score in pediatric patients with sepsis. Methods. This study is a prospective cohort involving sepsis patients under the age of 18 who were admitted to the PICU of Dr. Zainoel Abidin General Hospital from July to September 2024. PELOD-2 scores and routine blood tests were assessed on the first and third days of hospitalization. The correlation between variables was analyzed using Pearson’s correlation test. Results. A total of 44 subjects were included, with the majority being under 1 year old (31.8%), female (59.1%), and having respiratory infections as the most common primary infection source (27.3%). On day 1, 100% of patients had a PELOD-2 score ≥ 10, while on day 3, this percentage decreased to 63.6%. Most day 1 MPV values were within the 7.4–10.4 fL range (70.5%), while on day 3, 68.2% had MPV values > 10.4 fL. PDW values on both day 1 and day 3 were mostly within the normal range (10–18 fL), at 59.1% and 61.4%, respectively. There was a moderate correlation between day 3 MPV values and PELOD-2 scores (p = 0.06; r = 0.410), a moderate correlation between day 3 PDW values and PELOD-2 scores (p = 0.001; r = 0.518), and a moderate correlation between changes in PDW values and changes in PELOD-2 scores (p = 0.005; r = 0.471). Conclusion. Increases in MPV and PDW values, as well as changes in PDW, are correlated with PELOD-2 scores in children with sepsis.
