Abstrak latar belakang. transfusi darah yang sering pada anak talasemia beta mayor, menyebabkan kelebihan zat besi, sehingga terjadi hemosiderosis di seluruh jaringan tubuh (kulit, hepar dan ginjal), jika fungsi organ tersebut terganggu, menyebabkan gangguan metabolisme vitamin d. defisiensi vitamin d menyebabkan osteoporosis, patah tulang patologis, kelemahan otot, dan morbiditas yang diduga berkaitan dengan status hipokalsemia, yang berefek selama pertumbuhan. tujuan. untuk mengetahui hubungan status vitamin d dengan status kalsium pada anak dengan talasemia beta mayor di rsud dr. zainoel abidin banda aceh. metode. analitik observasional dengan pendekatan potong lintang, subjek anak talasemia beta mayor berusia 2-18 tahun di poliklinik talasemia anak rsud dr. zainoel abidin, sejak juli hingga november 2024. analisis data data kategorik dengan menggunakan uji spearman dengan nilai p 2000 ng/ml 70%. pada kelompok status defisiensi vitamin d, terbanyak pada laki laki 50%, usia anak 53,3%, hepatosplenomegali 50%. status hipokalsemia didominasi oleh laki-laki 81,8%, usia anak 86,6%, hepatosplenomegali 80% dan status feritin >2000 ng/ml 78,5%. subjek dengan status defisiensi vitamin d 94,4% nya mengalami hipokalsemia, koefisien korelasi 0,037 dan nilai p=0,017. kesimpulan. terdapat hubungan antara status vitamin d terhadap status kalsium pada anak talasemia beta mayor.

Abstract
Background. Frequent blood transfusions in children with beta-thalassemia major result in iron overload, leading to hemosiderosis in various body tissues (skin, liver, and kidneys). This condition can impair the function of these organs, disrupting vitamin D metabolism. Vitamin D deficiency contributes to osteoporosis, pathological fractures, muscle weakness, and morbidity, which are believed to be associated with hypocalcemia, a condition that significantly affects growth. Objective. To investigate the relationship between vitamin D and calcium status in children with beta-thalassemia major at dr. Zainoel Abidin Public Hospital in Banda Aceh, Indonesia. Methods. This analytic observational study employed a cross-sectional design involving children aged 2–18 years with beta-thalassemia major attending the Children's Thalassemia Polyclinic at dr. Zainoel Abidin Public Hospital in Banda Aceh, Indonesia. from July to November 2024. Categorical data were analyzed using the Spearman test with p-value2000 ng/mL in 70% subjects. Among those with vitamin D deficiency, 50% subjects were male, 53.3% subjects were children, and 50% subjects had hepatosplenomegaly. Hypocalcemia was predominantly observed in males (81.8%), children (92.3%), individuals with hepatosplenomegaly (80%), and those with ferritin levels >2000 ng/mL (78.5%). In subjects with vitamin D deficiency, 94.4% also had hypocalcemia (r = 0.037, p-value = 0.017). Conclusion. There is a significant association between vitamin D and calcium status in children with beta-thalassemia major.
