Pendahuluan: sindroma pasca coronavirus disease (covid-19) menjadi salah satu penyebab menurunnya kapasitas fungsional (vo2) dan produktivitas kerja khususnya pada tenaga kesehatan. mekanisme sindroma pasca covid-19 berkaitan dengan mekanisme inflamasi kompleks dan multistem. program rehabilitasi latihan jantung paru menjadi salah satu upaya untuk mempercepat proses pemulihan penderita sindroma pasca covid-19. penelitian ini bertujuan untuk menilai korelasi vo2 terhadap high sensitivity c-reactive protein (hscrp) dan neutrophil lymphocyte ratio (nlr) pada penderita sindroma pasca covid19 yang menjalani latihan jantung paru tersupervisi intensitas sedang dan tinggi. metode: studi ini termasuk studi observasional dengan desain kohor prospektif yang dilakukan pada ruang gym rehabilitasi jantung terpadu rumah sakit umum daerah dr. zainoel abidin banda aceh. latihan jantung paru tersupervisi dilakukan selama 6 minggu menyesuaikan dengan vo2 dasar peserta. analisis korelasi spearman’s dan pearson’s dilakukan dengan tingkat kepercayaan 95% hasil: sebanyak 30 individu (intensitas latihan: 19 sedang; 11 tinggi) intensi terlibat dalam penelitian ini dengan rerata usia secara berurutan 40,11 tahun dan 33,09 tahun berdasarkan intensitas latihan. pada kelompok intensitas sedang, rerata hscrp dan nlr adalah 3,3 mg/l dan 1,99 , sedangkan pada kelompok intensitas tinggi menunjukkan nilai 3,8 mg/l dan 1,79. tidak terdapat korelasi antara hscrp dan kapasitas fungsional pada kedua kelompok intensitas sedang dan intensitas tinggi. sebaliknya, nilai nlr menunjukkan koefisien korelasi -0,545 terhadap vo2 pada kelompok latihan intensitas sedang (p=0,016). kesimpulan: kapasitas fungsional berkorelasi negatif terhadap nlr dimana seiring dengan peningkatan kapasitas fungsional didapatkan penurunan nlr yang bermakna dengan kekuatan korelasi sedang pada penderita sindroma pasca covid-19 setelah menjalani latihan jantung paru tersupervisi pada kelompok intensitas sedang. kata kunci: sindroma pasca covid-19, latihan jantung paru tersupervisi, kapasitas fungsional, hscrp, nlr



Abstract

Introduction: Post coronavirus disease 2019 (COVID-19) syndrome is one of the causes of reduced functional capacity (VO2) and work productivity, especially for healthcare workers. The mechanism of the post COVID-19 syndrome is related to complex and multisystem inflammatory mechanisms. The cardiopulmonary exercise rehabilitation program is one of the efforts to improve the recovery process for patients of post COVID-19 syndrome. The aim of this study was to assess the correlation between high sensitivity C-reactive protein (Hs-CRP) and neutrophilto-lymphocyte ratio (NLR) with functional capacity (VO2max) and in individuals with post-COVID-19 syndrome who received moderate and high-intensity supervised cardiopulmonary exercise. Method: This study was observational with a prospective cohort design conducted at the Integrated Cardiac Rehabilitation Gym Installation of Dr. Zainoel Abidin Hospital, Banda Aceh, Indonesia. The supervised cardiopulmonary exercise was conducted for six weeks according to the participant's basic VO2max. Spearman's and Pearson's correlation analyses were used with a 95% confidence level to assess the correlations. Result: A total of 30 individuals (19 and 11 had moderate and high exercise intensity, respectively) were involved in this study. At moderate intensity, the average Hs-CRP and NLR were 3.3 mg/L and 1.99, respectively; while at high intensity, the values were 3.8 mg/L and 1.79, respectively. No significant correlation between Hs-CRP and functional capacity in moderate-intensity and high-intensity groups. In contrast, NLR was negatively correlated with functional capacity with moderate correlation (r=-0.545, p=0.016) in the moderate-intensity exercise group. Conclusion:NLR value was negatively correlated with functional capacity, there was a significant decrease in NLR in individuals with post-COVID-19 syndrome after receiving moderate intensity supervised cardiopulmonary exercise program.
