Prolanis adalah program pelayanan kesehatan bagi penderita penyakit kronis khususnya hipertensi dan diabetes melitus tipe 2 untuk mencapai kualitas hidup yang optimal. efektivitas prolanis sangat dipengaruhi oleh fasilitas pelayanan kesehatan, faktor pengetahuan dan keterlibatan pasien. keterlibatan pasien membutuhkan dukungan yang adekuat dari keluarga pasien dalam segi waktu, finansial dan emosional. penelitian ini bertujuan untuk mengetahui gambaran dukungan keluarga terhadap efektivitas prolanis di puskesmas batoh, banda aceh. penelitian dilakukan secara kualitatif menggunakan teknik wawancara semi terstruktur. pertanyaan wawancara dirancang dengan dasar menggunakan apgar (adaptability, partnership, growth, affection dan resolve) keluarga. peneliti melakukan wawancara terhadap sebelas responden utama yaitu keluarga pasien prolanis, dan delapan responden triangulasi yang tersiri dari pasien dan dokter keluarga di puskesmas batoh. penelitian ini menunjukkan bahwa keluarga pasien prolanis berperan dengan cara mendukung pengelolaan penyakit kronis pasien seperti memberi perhatian dan memantau kondisi kesehatan pasien, memberi pertolongan maksimal pada pasien, menjadi pmo pasien, dan berkerjasama dengan pasien dalam menghadapi penyakitnya, mendukung berbagai kegiatan pasien dalam menjalani program hidup sehat, meluangkan waktu berbagi cerita, meluangkan waktu untuk menemani pasien, menanggung semua kebutuhan pasien, berperan dalam pengaturan diet pasien dan semua hal tersebut berkaitan dengan pencapaian manfaat prolanis. didapatkan juga bahwa kendala penerapan pelayanan prolanis berkaitan dengan minimnya edukasi yang disampaikan oleh petugas kesehatan ke keluarga pasien secara langsung dan bahwa program kegiatan prolanis sudah efektif bagi pasien dan keluarganya.

Prolanis is a health service program for sufferers of chronic diseases, especially hypertension and type 2 diabetes mellitus, to achieve optimal quality of life. The effectiveness of Prolanis is greatly influenced by health service facilities, knowledge factors and patient involvement. Patient involvement requires adequate support from the patient's family in terms of time, finances and emotions. This study aims to determine the description of family support for the effectiveness of Prolanis at the Puskesmas Batoh, Banda Aceh. The research was conducted qualitatively using semi-structured interview techniques. Interview questions were designed on the basis of using the family APGAR (Adaptability, Partnership, Growth, Affection and Resolve). Researchers conducted interviews with eleven main respondents, namely the families of Prolanis patients, and eight triangulated respondents consisting of patients and family doctors at the Puskesmas Batoh. The research show that the majority of families play a role in supporting the management of the patient's chronic disease, such as paying attention to and monitoring the patient's health condition, providing maximum assistance to the patient, being the patient's PMO, and collaborating with the patient in dealing with the disease, supporting various patient activities in carrying out a healthy living program, taking the time to share stories, taking the time to accompany the patient, taking care of all the patient's needs, playing a role in managing the patient's diet and all of these things are related to achieving the benefits of prolanis. It was also found that the obstacles to implementing Prolanis services were related to the lack of education delivered by health workers to patient families directly and that the Prolanis activity program was effective for patients and their families.
