Hipertensi pada dewasa dan lansia menjadi fokus asuhan keperawatan komunitas mengingat urgensinya sebagai penyakit kronis dan berisiko menimbulkan komplikasi kardiovaskular. kelompok ini memiliki beban penyakit tinggi dan memerlukan pengelolaan jangka panjang, sementara pemanfaatan intervensi nonfarmakologis masih belum optimal. studi kasus ini bertujuan menguraikan penerapan asuhan keperawatan komunitas pada agregat dewasa dan lansia dengan hipertensi di kecamatan darussalam, aceh besar. hasil pengkajian melalui data sekunder, focus group discussion (fgd), dan kuesioner menunjukkan bahwa rendahnya pengetahuan dewasa dan lansia dalam pengendalian hipertensi, kurangnya pemeriksaan tekanan darah rutin, serta ketidakpatuhan terhadap terapi farmakologis dan pemanfaatan prolanis. kondisi ini menyebabkan pengendalian hipertensi belum optimal dan menegaskan perlunya penerapan terapi nonfarmakologis. berdasarkan hasil pengkajian tersebut, ditetapkan diagnosa keperawatan berupa ketidakefektifan manajemen mandiri kesehatan. perencanaan keperawatan disusun mengacu pada nursing outcomes classification (noc), yaitu peningkatan pengetahuanmdan perilaku kesehatan, serta nursing interventions classification (nic) melalui pendidikan kesehatan dan demonstrasi terapi pijat refleksi kaki. implementasi keperawatan yang dilaksanakan berupa penyuluhan dan demonstrasi teknik pijat refleksi kaki. hasil evaluasi menunjukkan adanya peningkatan pengetahuan yang ditandai dengan skor pre-test (43%) dan skor post-test (71,4%) serta penurunan tekanan darah sistolik dan diastolic 3-5 mmhg setelah diberikan terapi pijat refleksi kaki. temuan ini menunjukkan bahwa terapi pijat refleksi kaki merupakan intervensi keperawatan komunitas nonfarmakologis yang aman dan efektif dalam membantu pengendalian hipertensi pada dewasa dan lansia.

Abstract
Hypertension in adults and the elderly is a focus of community nursing care given its urgency as a chronic disease and the risk of developing cardiovascular complications. This group has a high disease burden and requires long-term management, while the utilization of non-pharmacological interventions is still suboptimal. This case study aims to describe the implementation of community nursing care in an aggregate of adults and elderly with hypertension in Darussalam District, Aceh Besar. The results of the assessment through secondary data, Focus Group Discussions (FGDs), and questionnaires indicate that adults and elderly have low knowledge in controlling hypertension, lack of routine blood pressure checks, and non-compliance with pharmacological therapy and the use of prolanis. These conditions result in suboptimal hypertension control and emphasize the need for the implementation of non-pharmacological therapy. Based on the results of the assessment, a nursing diagnosis was determined as ineffective self-management of health. Nursing planning was prepared referring to the Nursing Outcomes Classification (NOC), namely improving health knowledge and behavior, and the Nursing Interventions Classification (NIC) through health education and demonstrations of foot reflexology therapy. The nursing implementation carried out included counseling and demonstrations of foot reflexology techniques. The evaluation results showed an increase in knowledge, indicated by pre-test scores (43%) and post-test scores (71.4%), as well as a 3-5 mmHg decrease in systolic and diastolic blood pressure after foot reflexology therapy. These findings demonstrate that foot reflexology therapy is a safe and effective non-pharmacological community nursing intervention in helping control hypertension in adults and the elderly.
