ABSTRAK
Evaluasi kejadian medication error telah dilakukan pada resep pasien anak rawat inap di RSUD dr. Zainoel Abidin Banda Aceh. Penelitian ini bertujuan untuk menjelaskan gambaran kejadian medication error yang terjadi, menentukan persentase angka kejadian pada setiap fase dan menentukan fase dengan angka kejadian tertinggi. Penelitian ini merupakan penelitian observatif dengan metode survei deskriptif. Pengambilan sampel dilakukan dengan teknik total sampling selama satu bulan dan diperoleh sampel sebanyak 83 lembar resep. Hasil yang diperoleh menunjukkan bahwa kejadian medication error terjadi pada seluruh sampel penelitian (n=83). Persentase kejadian tertinggi didapatkan pada fase transcribing (67,01%) dan persentase kejadian pada fase lainnya yaitu fase prescribing (31,72%), fase dispensing (0,12%), dan fase administering (1,15%). Medication error tertinggi pada fase prescribing, fase transcribing, fase dispensing, dan fase administering masing-masing terjadi pada parameter tidak adanya penulisan tinggi badan pasien, tidak lengkap atau tidak adanya penulisan kekuatan sediaan obat kesalahan pada saat pengambilan obat dengan kekuatan sediaan yang tidak tepat, dan obat yang tersisa di samping bed atau loker pasien.

ABSTRACT
Evaluation of medication error has been done in pediatric inpatient’s prescriptions at dr. Zainoel Abidin Banda Aceh. The purposes of this study are to explain the incidence of medication errors that occurred on pediatric inpatient’s prescriptions, to determine the percentage of each phases, and to determine the highest percentage of medication error. This study is an observative study with descriptive survey method. The sampling was done by the total sampling technique for one month and a total of 83 prescriptions were obtained. The result showed that the incidence of medication errors was occurred in all samples of this study (n = 83). The highest percentage of the incidence was found in the transcribing phase (67.01%) and the percentage of the other phases were prescribing phase (31.72%), dispensing phase (0.12%), and administering phase (1.15%). The highest medication error of each phases that frequently occurred were incomplete writing of patient’s height, incomplete or incorrect writing of the dosage strength, error in taking the drugs by improper dosage strength, and the remaining medicine alongside the bed or patient’s locker.
