ABSTRAK

Dokumentasi keperawatan merupakan informasi tertulis yang dihasilkan dari proses keperawatan kepada pasien. Dokumentasi yang baik mencerminkan keakuratan dalam memberikan pelayanan. Supervisi yang tidak adekuat, kompetensi, kepercayaan diri, serta motivasi yang rendah menjadi kendala dalam pendokumentasian keperawatan. Tujuan penelitian untuk mengetahui faktor yang berhubungan dengan pendokumentasian keperawatan di Rumah Sakit Umum Daerah dr. Zainoel Abidin Banda Aceh. Jenis penelitian ini adalah kuantitatif; survey analitik dengan pendekatan cross sectional study. Alat pengumpulan data berupa kuesioner yang dibagikan kepada responden serta melakukan observasi rekam medis pasien di ruang rawat. Jumlah sampel yaitu 185 perawat dan 185 rekam medis yang didapatkan menggunakan tehnik purposive sampling. Analisis data menggunakan uji chi square dan uji regresi logistik. Hasil menunjukkan bahwa variabel yang berhubungan terhadap pendokumentasian keperawatan adalah supervisi (p-value=0,000) dan motivasi (p-value=0,008). Sedangkan variabel yang tidak memiliki hubungan adalah kompetensi (p-value=0,129) dan kepercayaan diri (p-value=1,000). Adapun faktor yang paling berhubungan dengan pendokumentasian keperawatan adalah supervisi dengan p-value 0,001 dan OR 6,052 yang memberikan kesimpulan bahwa dengan pelaksanaan supervisi yang baik memiliki kemungkinan 6 kali lebih besar dalam meningkatkan pendokumentasian keperawatan. Supervisi berkala, pelatihan serta adanya teknologi komputerisasi terkait pendokumentasian dapat meningkatkan kualitas dalam pendokumentasian keperawatan.

ABSTRACT Nursing documentation is written information about nursing processes implemented on patients in a hospital. Good nursing documentation reflects the accuracy in providing nursing care. Supervision, competence, self-confidence, and motivation become an obstacle in nursing documentation. The objective of this study was to determine factors associated with nursing documentation in dr. Zainoel Abidin Regional Public Hospital of Banda Aceh. This quantitative study was conducted by means of an analytic survey with a cross-sectional study approach. The data were collected by distributing a questionnaire to each respondent and observing the medical records of the patients in the wards. A number of 185 nurses and 185 medical records were chosen as the samples through a purposive sampling technique. The stage of data analysis used the chi-square test and the logistic regression test. The results showed that the variables associated with nursing documentation were the variables of supervision (p-value=0.000) and motivation (p-value=0.008). On the other hand, the variables that had no relationship with nursing documentation at the hospital were the variables of competence (p-value = 0.129) and self-confidence (p-value = 1.000). The most related factor was the variable of supervision which obtained a p-value of 0.001 and an OR value of 6.052 which indicated that the implementation of good supervision had a possibility of improving nursing documentation six times greater than any other variable. Periodic supervision, training and the existence of computerized technology related to documentation can improve the quality of nursing documentation.
