PENGARUH TERAPI EKSTRAK DAUN SEURAPOH (Chromolaena odorata) TERHADAP INDEKS ULKUS, GAMBARAN HISTOPATOLOGI ULKUS GASTER DAN KADAR TUMOR NECROSIS FACTOR ALPHA (TNF-α) YANG DIINDUKSI ASPIRIN PADA TIKUS PUTIH (Rattus norvergicus)

Latar Belakang
Ulkus Gaster adalah gambaran luka dengan kedalaman hingga ke sub mukosa lambung yang terjadi akibat terputusnya integritas mukosa lambung oleh asam klorida atau pepsin. Aspirin dosis rendah menyebabkan efek sistemik terhadap mukosa lambung dan menyebabkan ulkus gaster yang dihubungkan dengan Tumor Necrosis Factor alpha (TNF-α). Daun tumbuhan Seurapoh atau Chromolaena odorata memiliki efek perlindungan dari ulkus gaster terutama yang diinduksi oleh Aspirin, sehingga penelitian ini bertujuan untuk mengetahui pengaruh pemberian ekstrak daun Seurapoh (Chromolaena odorata) terhadap Indeks Ulkus, Histopatologi ulkus gaster dan kadar TNF-α pada ulkus gaster yang diinduksi oleh Aspirin pada tikus putih.

Metode
Penelitian eksperimental dengan menggunakan model ulkus gaster tikus putih (Rattus norvergicus) tipe Wistar ini dilaksanakan pada Juni 2021-Juli 2022 di Laboratorium Fakultas Kedokteran Hewan Universitas Syiah Kuala. Daun Chromolaena odorata dikumpulkan dari beberapa lokasi berbeda dan ekstraksi dilakukan di Laboratorium Fakultas Matematika dan Ilmu Pengetahuan Alam Universitas Syiah Kuala dengan menggunakan metode ekstraksi etanol. Tikus putih dibagi menjadi 5 kelompok perlakuan; kontrol negatif, ekstrak 50 mg/kilogram berat badan (kgbb), 100 mg/kgbb, 200 mg/kgbb dan kelompok kontrol positif yang diberikan obat omeprazole 20 mg. Semua tikus mendapatkan Aspirin 400 mg/kgbb satu dosis. Setelah 3 hari pemberian Aspirin, tikus putih kemudian diberikan ekstrak dan omeprazole. Pada hari ke-14 setelah pemberian ekstrak dan omeprazole, Indeks ulkus dan gambaran histopatologi dengan pewarnaan Hematoksilin-Eosin dilakukan dengan pemeriksaan makroskopis dan mikroskopis. Kadar TNF-α mukosa lambung dianalisa menggunakan metode Enzyme-linked immunosorbent assay (ELISA).

Hasil
Dari uji awal diketahui bahwa ekstrak geotermal lebih superior dalam memperbaiki ulkus gaster akibat pemberian aspirin pada tikus, sehingga digunakan pada penelitian ini. Penelitian ini menunjukkan perbedaan indeks ulkus antar kelompok perlakuan pada hari ke-14, tetapi tidak terdapat perbedaan skor integritas epitel mukosa pada kelompok perlakuan tersebut. Lebih lanjut penelitian ini juga tidak dapat menemukan perbedaan kadar TNF- dalam jaringan ulkus pada kelima kelompok perlakuan.

Kesimpulan
Terdapat perbedaan indeks ulkus yang bermakna secara statistik pada tikus yang diberikan ekstrak daun Chromolaena odorata dibandingkan dengan kontrol negatif dan kontrol positif pada Tikus putih yang diinduksi Aspirin, tetapi tidak terdapat pengaruh pemberian ekstrak daun Chromolaena odorata terhadap gambaran histopatologi ulkus gaster dan kadar TNF-α pada Tikus putih yang diinduksi Aspirin.

ABSTRACT EFFECT OF SEURAPOH (Chromolaena odorata) LEAF EXTRACT THERAPY ON ULCER INDEX, HISTOPATHOLOGY AND TUMOR NECROSIS FACTOR ALPHA (TNF-α) LEVEL OF ASPIRIN-INDUCED GASTRIC ULCER IN WHITE RATUS (Rattus norvergicus) Introduction Gastric ulcer is a depth to sub-gastric mucosa wound that occurs due to disruption of gastric mucosa integrity by hydrochloric acid or pepsin. Low-dose aspirin causes systemic effects on the gastric mucosa and lead to gastric ulcers associated with Tumor Necrosis Factor alpha (TNF-α). Seurapoh or Chromolaena odorata leaves have a protective effect from gastric ulcers, especially those induced by Aspirin. Therefore, this study aimed to determine the effect of Chromolaena odorata leaf extract therapy on Ulcer Index, Gastric ulcer histopathology and TNF-α level in Aspirin-induced gastric ulcers. Method This experimental study using Wistar type white mouse gastric ulcer model (Rattus norvergicus) was carried out in June 2021-July 2022 at the Laboratory of the Faculty of Veterinary Medicine, Syiah Kuala University. Chromolaena odorata leaves were collected from several different locations and the extraction was carried out at the Laboratory of the Faculty of Mathematics and Natural Sciences, Syiah Kuala University using the ethanol extraction method. White rats were divided into 5 treatment groups; negative control, extract 50 mg/kilogram body weight (kgbw), 100 mg/kgbw, 200 mg/kgbw and the positive control group were given omeprazole 20 mg. All rats received aspirin 400 mg/kgbw in one dose. After 3 days of aspirin administration, white rats were then given extract and omeprazole. On the 14th day after administration of extract and omeprazole, ulcer index and histopathological appearance with Hematoxylin-Eosin staining were performed by macroscopic and microscopic examination. Gastric mucosal TNF-α levels were analyzed using the Enzyme-linked immunosorbent assay (ELISA) method. Results The preliminary study showed that geothermal extract was superior in improving Aspirin-induced gastric ulcers in rats, so it was used in this study. This study showed statistically significant differences in the ulcer index between treatment groups on day 14, but there was no difference in the score of mucosal epithelial integrity. Furthermore, this study also could not find differences in TNF-α levels in ulcer tissue in the five treatment groups. Conclusion There was a statistically significant difference in the ulcer index in rats after giving Chromolaena odorata leaf extract for 14 days compared to negative control and positive control in Aspirin-induced white rats, but there was no effect of Chromolaena odorata leaf extract on the histopathology of gastric ulcers and TNF-α levels in Aspirin-induced gastric ulcer.
