Nyeri Punggung Bawah (NPB) merupakan rasa nyeri dari tepi bawah iga sampai tepi atas gluteus superior yang dapat dirasakan menjalar atau tidak. Pada NBP, patofisiologi nyeri (nosiseptif, neuropati, sensitisasi sentral) menentukan terapi sehingga penting untuk dilakukan skrining awal untuk menentukan patofisiologi nyeri. Namun, untuk jenis nyeri nosiseptif dan sensitisasi sentral belum terdapat instrument skrining yang akurat dan teruji sebagai gold standard. Tujuan penelitian ini adalah menyusun screening tool untuk nyeri nosiseptif dan sensitisasi sentral pada NPB. Penelitian ini merupakan studi deskriptif dengan metode wawancara terhadap 61 responden dari Poliklinik Saraf RSUDZA Banda Aceh pada periode Januari-Maret 2022. Analisis data menggunakan uji validitas, reliabilitas, dan uji diagnostik. Hasil penelitian menunjukkan NPB lebih sering dialami oleh perempuan dibanding laki-laki, dan lebih sering pada usia 36-45 tahun. Hasil uji screening tool nyeri nosiseptif didapatkan 6 dari 8 pertanyaan valid (r > 0,5); 4 dari 6 pertanyaan reliabel (Alpha Cronbach > 0,6); dan pada uji diagnostik didapatkan cut off point 4 (sensitivitas 57,89%, spesifisitas 78,26%, PPV 81,48%, NPV 52,94%, dan akurasi 65,57%). Sedangkan hasil uji screening tool nyeri sensitisasi sentral didapatkan 11 dari 13 pertanyaan valid; 6 dari 11 reliabel; dan pada uji diagnostik didapatkan cut off point 12 (100%, spesifisitas 89,83%, PPV 25%, NPV 100%, dan akurasi 90,16%). Screening tool nyeri nosiseptif cukup baik digunakan pada pelayanan pasien NPB, namun screening tool nyeri sensitisasi sentral masih diperlukan penelitian lebih lanjut.

Low back pain (LBP) is pain sensation from the lower edge of the ribs to the upper edge of the superior gluteus which can be radiate or not. In LBP, pain pathophysiology (nociceptive, neuropathy, central sensitization) determines the therapy, so it is important to do initial screening for the pain. Unfortunately, for nociceptive pain and central sensitization, there is no accurate and tested screening instrument as the gold standard. This study aims were to develop a screening tool for nociceptive pain and central sensitization pain in LBP. This is a descriptive study by interviewing 61 respondents from the Neurology Polyclinic of RSUDZA Banda Aceh in the period January-March 2022. Data analysis uses validity, reliability, and diagnostic tests. The results showed that LBP was more often experienced by women, and more often at the age of 36-45 years. Nociceptive pain screening tool test result showed 6 out of 8 questions were valid (r > 0,5); 4 of 6 questions were reliable (Cronbach's Alpha > 0.6); and the diagnostic test showed cut off point 4 (sensitivity 57.89%, specificity 78.26%, PPV 81.48%, NPV 52.94%, and accuracy 65.57%). Meanwhile, central sensitization pain screening tools test results showed 11 out of 13 questions were valid; 6 out of 11 questions were reliable; and the diagnostic test showed a cut-off point of 12 (100%, specificity 89.83%, PPV 25%, NPV 100%, and accuracy 90.16%). Nociceptive pain screening tools are quite well used for patients with NPB, however further research is needed for central sensitization pain screening tools.
