Readmisi rumah sakit memberikan dampak negatif terhadap kualitas hidup pasien dan meningkatnya biaya perawatan dan kesehatan. faktor-faktor resiko penyebab terjadinya readmisi pada pasien diabetes mellitus tipe 2 terdiri dari usia, jenis kelamin, komplikasi diabetes mellitus, ketidak-stabilan kadar gula darah, strategi edukasi, pengaturan diet, latihan fisik, dan kepatuhan terhadap program pengobatan. penelitian ini bertujuan untuk mengetahui determinan readmisi pasien diabetes mellitus tipe 2 di rumah sakit umum cut meutia kabupaten aceh utara. jenis penelitian adalah kuantitatif survey analitik dengan menggunakan desain cross sectional study. jumlah sampel penelitian 184 pasien readmisi dengan diabetes mellitus tipe 2 yang dipilih melalui teknik accidental sampling. pengambilan data dilakukan melalui penyebaran angket dan telaah rekam medik pasien. alat pengumpulan data terdiri dari the diabetes early readmission risk indicators (derri™), hasil pemeriksaan laboratorium kadar gula darah (kgd), strategi edukasi diabetes mellitus, united kingdom (uk) diabetes and diet questionnaire, international physical activity questionnaire (ipaq), insulin treatment appraisal scale (itas), dan kuesioner readmisi. data dianalisis menggunakan uji chi-square dan regresi logistik berganda. hasil penelitian menunjukkan ada hubungan usia (p=0,013), tidak ada hubungan jenis kelamin (p=0,073), ada hubungan komplikasi diabetes mellitus (p=0,000), ada hubungan ketidak-stabilan kadar gula darah (kgd) (p=0,000), ada hubungan strategi edukasi diabetes mellitus (p=0,000), ada hubungan pengaturan diet (p=0,000), tidak ada hubungan latihan fisik (p=0,430) dan ada hubungan kepatuhan terhadap program pengobatan dengan readmisi pasien diabetes mellitus tipe 2 (p=0,000). komplikasi diabetes mellitus merupakan faktor yang paling berhubungan dengan readmisi (or=5,330). diharapkan kepada responden untuk meningkatkan kemandirian dalam perawatan diri dan pihak rumah sakit untuk mengevaluasi kembali dan menyusun strategi pelayanan dan intervensi keperawatan melalui discharge planning untuk pencegahan komplikasi atau mengurangi readmisi pasien diabetes mellitus tipe 2.

Abstract
Hospital readmissions have many negative impacts on patient’s life quality and increase health care costs. Factors leaded to readmission among type 2 were age, gender, diabetic complications, blood glucose level instability, education strategy, physical exercise and medical therapy programme adherence. This study aims to explore and analyse the determinants of type 2 diabetic patients’ readmission in Cut Meutia General Hospital in North Aceh Regency. This study is a correlational cross-sectional study. Sample of 184 type 2 diabetic readmission patients selected through accidental sampling technique. Data collecting was performed by questionnaire delivery and patient’s medical records review. Data collection instruments included demographic data, The Diabetes Early Readmission Risk Indicators (DERRI™), blood glucose level report, education strategy on diabetes mellitus, United Kingdom (UK) Diabetes and Diet Questionnaire, International physical activity questionnaire (IPAQ), Insulin Treatment Appraisal Scale (ITAS), and the questionnaire about readmission. Data were analysed using uji Chi-square test and multiple linear regression. The result showed the association between age (p=0,013), the disassociation between gender (p=0,073), the association between complications (p=0,000), the association between blood glucose level instability (p=0,000), the association between education strategy (p=0,000), the association between diet control (p=0,000), the disassociation between physical exercises (p=0,053), the association between therapy programme adherence and readmission (p=0,000). While complications were the most associated variable to readmission (OR=5,330). It is recommended to respondents to improve selfcare in diabetic management and to the hospital management to evaluate and design health care and nursing interventions strategy to prevent diabetic complications and reduce hospital readmission through discharge planning among type 2 diabetic patients.
